We participate with Medicare, Blue Cross/Blue Shield, Priority Health, BlueCare Network, United HealthCare, PPOM and several other insurance companies. Your insurance carrier is the best resource for your policy benefits and authorization requirements.  Any required authorization for services must be obtained prior to your appointment or payment may be denied.  As a courtesy, we will bill your insurance claim for you.

Office visit co-payment is due at the time of service.  For your convenience, we accept cash, checks and Visa/MasterCard.
