Scheduling an Appointment

Please call any one of the following offices during business hours to schedule your appointment at that location.

If the services you require are not available at the location nearest you, you may redirected to the appropriate site for scheduling.

Your insurance carrier may require you to have prior authorization from your primary care physician prior to scheduling your appointment.  Your primary care physican will provide us with information which helps us determine which services you may require during your appointment, and will sometimes assist in scheduling the appointment.

If you are currently under the care of a West Michigan Heart Cardiologist, you may call us to schedule follow up visits and tests as instructed by your cardiologist.  Please check with your insurance carrier to understand whether another referral from your primary care physician is required for services.

In the event you need to reschedule your appointment, we would greatly appreciate a 24-hour notice of cancellation so that we may accommodate another patient at that time.   Please contact the office where your appointment is scheduled so that we can cancel the appointment and work with you to create a new appointment at a more convenient time.  If you contact us after hours to cancel an appointment, please leave a message on our after hours voice mailbox.
